We look forward to meeting you!

After making your first appointment with KFI, you will receive a welcome packet via email. This packet contains information confirming your appointment, directions to our state-of-the-art center, and important medical forms that will need to be completed prior to your first appointment.

On the day of your appointment, we ask that you arrive 15 minutes early. Please complete and bring the following to your appointment:

• Insurance card and insurance coverage information

• Driver’s license (or photo ID)

• Completed forms

  • Patient Demographics

  • Medical Record Release

  • Consent forms

• Please make sure to complete your medical history in our patient portal (and your partner’s, if applicable) prior to your appointment

• Insurance co-payment or self payment

  • We accept cash, personal check, MasterCard or Visa

What to Expect

Much of the first appointment is spent reviewing your past medical history and family-building goals. We need you (and your partner, if applicable) to complete your KFI record release at least two weeks prior to your first appointment to allow us time to obtain your prior medical records. You may also bring your medical records/information to your first appointment if you cannot access them beforehand.

During the meeting with your provider, you will have the opportunity to ask any questions that you may have. Some patients find it helpful to write questions down in advance of the appointment to make sure they don’t forget to ask during the appointment. Initial consultations are often most productive if your partner can also be present (if applicable) to ensure that everyone hears the same information and all questions are appropriately addressed. Commonly, the initial consultation will conclude with your provider making recommendations for additional testing for you (and your partner, if applicable) that can be scheduled to help better understand the types of issues that may be causing your infertility, as well as what types of therapeutic options may be most valuable in helping you to reach your reproductive goals. Test results and personalized treatment plans are typically reviewed in detail at a follow-up visit once all testing has been completed.
